Thanks for your interest in referring a client to this service. Although clients do not require a referral to be seen, a GP referral through the ‘Chronic Disease Management Scheme‘ allows access to up to 5 subsidised sessions per calendar year. The rebate is $61.80 per session. More information on pricing can be found under the ‘pricing’ tab.

Collaboration and keeping you informed about the individuals health is an important part of this service. After the initial assessment and final session a comprehensive report will be sent through to you.
